Our Process

Our process has been developed based on the pharmaceutical care model and is consistently implemented to meet your needs.

We adhere to the American College of Clinical Pharmacy Standards of Practice for Clinical Pharmacists and the Alberta College of Pharmacists Code of Ethics and Standards of Practice for Pharmacists and Pharmacy Technicians.

To begin, we gather important medication and health information from you and your health care team. Together we gain an understanding of your unique health circumstances and your health goals.

Next we develop mutual goals for your medication therapy. Once the goals have been established we discuss potential or actual medication therapy problems that have been identified.

The Clinical Pharmacist then comes up with a detailed plan on how to solve the medication therapy problems. The plan is implemented, records of care are created and communication occurs with other healthcare professionals.

Finally a follow-up plan is scheduled to ensure you are on track to achieve your health goals.

I recently underwent a major surgery in Edmonton, Alberta. I had a double lung transplant and as I went through the adjustments of the after care program things were overwhelming.
